Assignment Overview
- Juneau, Alaska 99801
- 13-week W-2 contract; 1 opening
- Monday–Friday, 8 a.m.–5 p.m..
- 40 guaranteed hours per week, subject to final assignment terms.
- Start is subject to fingerprint clearance and facility confirmation, at least 6 weeks after fingerprint cards are received. Housing and transportation are not provided by the facility.
Your Work
Help patients connect the pieces of their care. This role combines nursing judgment, education and follow-through as you work with patients, families and the clinical team to make treatment plans easier to understand and navigate.
Provide outpatient nursing assessment, care planning, patient education and case management, coordinate referrals and follow-up and collaborate with the clinic team on safe, culturally responsive care.
What You Bring
- Active unrestricted Alaska RN license at submission and graduation from an accredited nursing program; bachelor’s degree preferred.
- At least one year of case-management experience within the last two years in a hospital, outpatient or ambulatory setting.
- Current BLS; case-management certification preferred.
